1. Submit payment using the store above.
2. Complete the below registration form and provide your invoice number (provided to you when payment was made) in the "Further Details" section of the registration.
3. If mailing in check or paying cash to your local representative, provide that information in the "Further Details" portion of the registration.
4. Payment alone does not constitute registration, the below form must be submitted as well. If enrolling in payment plan, state this in the "Further Details" section. Registration form is only submitted once, regardless of payment plan/option.
